Constant Care Management Company, an affiliate and sister company of The LaSalle Group, oversees all aspects of management and maintenance of the Autumn Leaves communities, including healthcare and nursing, dietary operations and life enrichment programs, family support groups, educational seminars and ongoing feedback to help families better understand their loved one’s situation. Constant Care Management is also responsible for day-to-day operations at the Autumn Oaks assisted living community.
